Conditions of Enrollment

As a Ho’ala member, I hereby agree: (a) that the Ho’ala Dental Program is not an insurance policy; it is an agreement for the provision of dental services at a discounted price; (b) that discounted fees provided through the Ho’ala Dental Program are only for services provided at Hawaii Family Dental Centers locations; (c) that the benefits of the Ho’ala Dental Program do not coordinate with any insurance plan; (d) to abide by the payment schedule published at the time of enrollment (fee increases may occur on an annual basis); (e) to pay the out-of-pocket costs at the time the services are received; (f) that there shall be no refunds given for any cancellation; and (g) that the Ho’ala Dental Plan Coordinator shall set the date on which the dental program enrollment shall begin.

Effective and Termination Date

Coverage becomes effective on the first day of the period or the date the application and full premium is postmarked, whichever is later. Coverage terminates at the end of the period through which premium is paid. Dependent coverage will not be effective prior to that of the member. Coverage is available only in Hawaii. There shall be no refunds given for any cancellation from the plan.

Termination of membership

We reserve the right to terminate a membership for any of the following reasons:

• member no longer meets the criteria for membership.
• member fails to make a required payment when due.
• member fails to pay the payment fees for dental services received.
• member engages in conduct that hampers our ability to provide dental services.
• member commits a fraudulent act to obtain benefits under the Ho’ala Dental Program.
