Plan Description

The Ho’ala Individual Plan is a cost-effective program that provides its members with certain privileges, such as discounted prices on dental services. Participants pay an annual membership fee and are then entitled to a host of discounts and benefits. Our network of providers are located at ten convenient centers statewide. Quality is never compromised, so you and your family will always get the best dental care possible.

Online Enrollment

Enrolling for Ho’ala online is quick and easy. Click the “Enroll Now” button, fill out the online application, select one of our three plan options, and make your payment.

The three plan options include

Single (plan coverage for one person) - $192
Two-party (plan coverage for two people) - $384
Family (plan coverage for three or more people) - $672

Membership fees in this program are pre-paid for a twelve-month period. You will be sent a membership card in the mail approximately one week following the receipt of your payment. The membership card is proof of membership in the Ho’ala Dental Program and must be presented at any Hawaii Family Dental Centers location at the time of your visit. Lost cards should be immediately reported so that a replacement membership card may be issued.

Using Your Benefits

You are eligible to begin using services the first day of the following month. For example, if you made your payment on January 6th, you are eligible to begin using services on February 1st.

Call the dental center of your choice from our list of network providers and schedule an appointment. Your fixed out of pocket costs will be presented to you and collected the day you receive services.

Co-payment Schedule

Each member shall be entitled to the dental services set forth in the Copayment Schedule.

All copayments for services are to be paid directly to the dental office by the patient, and will be collected at the time service is rendered. There is no maximum benefit dollar amount for dental services received under the program.
